<HARD-GATE>
The following rules are NON-NEGOTIABLE:
- NO code changes before Phase 4. Phases 1-3 are read-only operations on the target project.
- NO phase advancement without explicit user approval. Each phase gate requires user confirmation.
- Analysis report (Phase 1) MUST be written to disk. It captures institutional knowledge about the codebase.
- Review report (Phase 2) MUST be written to disk. It documents all findings for planning.
- Plans guide Phase 4, but professional judgment is allowed. Execute the plans' intent faithfully. You MAY add type annotations, defensive checks, meaningful names, and idiomatic patterns. You MUST NOT add features, endpoints, or behaviors not in the plan.
- NO skipping phases. Even if the user says "just fix it", go through all 4 phases.
- PRESERVE working functionality. Never break existing passing tests or working features during execution. The Preservation Gate enforces this.
- Production-grade improvements. When improving existing code, target production-ready quality — not MVP patches. Security hardening, observability, error resilience, and operational readiness are part of every improvement, not optional extras.

A deep-thinking protocol available at critical decision points. Operates in two tiers:
Default: Lightweight Mode (always runs) At each decision point (BS-1 through BS-3), structured self-reflection inline — no sub-agent calls.
Steps: Research (1-2 web searches) → Multi-Perspective self-evaluation (6 roles, inline) → Self-Interrogation (3 challenges) → Synthesis.
On-demand: Full Mode (user opt-in) When the user wants deeper analysis, adds sub-agent-based alternative generation, quality gate, and independent audit.
Decision points:
- BS-1 (Phase 1→2): Analysis completeness check — does the analysis cover everything needed for the user's goal?
- BS-2 (Phase 2): Review scope validation — is the review targeting the right areas?
- BS-3 (Phase 3): Task decomposition review — are tasks properly scoped and ordered?
- BS-7 (Phase 4): Error recovery — user-opt-in only

On skill invocation, FIRST check if .project-surgeon/ directory exists in the current directory.
- If `.project-surgeon/` does not exist: Fresh start. Ask user for an instance name, create
.project-surgeon/<name>/state.json, begin Phase 1. - If `.project-surgeon/` exists:
- Check for legacy
.project-surgeon/state.json(old single-instance format at root). If found, offer migration: rename to.project-surgeon/<chosen-name>/state.json. - List all subdirectories (each is a surgeon instance). Present current phase + last-updated for each.

Phase 1: Analysis — 项目分析
Goal: Deep automated scan of the existing project producing a comprehensive understanding report.
Protocol:
- Project Discovery Scan — directory tree, file statistics, tech stack detection from manifest files
- Architecture Pattern Detection — infer architecture (MVC/layered/microservices/etc.), entry points, module dependencies
- Dependency Health Check — lockfile analysis, native audit tools (npm audit/pip audit/etc.), outdated version detection
- Documentation & Configuration Inventory — README, CI configs, Docker, IaC, and their freshness
- Generate Report + Collect User Goal — write

Phase 2: Review — 7 维度审查
Goal: Systematic code review using the Bug Fixer 7-dimension protocol, tailored to Phase 1 findings.
Protocol:
- Scope Determination + BS-2 — based on user goal, determine scan scope and dimension priorities
- Execute 7-Dimension Review — tiered scanning (Grep→Read→Deep), 40-file budget. Dimensions: Security, Logic, Concurrency, Performance, Error Handling, Dependencies, Consistency
- Cross-Reference with Phase 1 — correlate findings with architecture analysis, identify systemic issues and hot spots
- Generate Review Report — write
