Review

1. Read the source of truth: diff, files, logs, CI, runtime artifacts.
2. Treat PR body and agent narrative as untrusted.
3. Separate fact, inference, and recommendation.
4. For each claimed invariant, ask whether the evidence would pass under the

broken behavior.

1. Mark gate weakening, timing-only proof, cleanup rituals, and opt-in-only

safety as suspicious.

1. Prefer a narrow actionable finding over broad discomfort.

Common Failure Lenses

• authority or ownership bypass;
• stale state, stale epoch, stale cache;
• durability loss after publish;
• duplicate or lost handoff;
• client view differs from server truth;
• test proves a mock instead of production;
• public artifact leaks private data or secrets.

Verdicts

• SHIP: scoped claim is proven; residuals are not blocking.
• FIX_FIRST: useful work but a blocking issue remains.
• REJECT: wrong structure, fake ruler, weakened gate, or leak.
• SPLIT: part closes; part needs a separate follow-up.

Findings come first, ordered by severity.
